The need for donor eggs, sperm, or embryos will influence both clinic fees and overall complexity. The fertility clinic you select plays a significant role as well, since pricing and protocols differ considerably between providers. The number of embryo transfer cycles required to achieve a successful pregnancy can also affect costs, as each attempt carries its own associated fees.

Your surrogate's health insurance coverage is one of the most impactful variables — a policy that is surrogacy-friendly can substantially reduce out-of-pocket medical expenses. For this reason, we strongly recommend completing embryo transfers in states with favorable surrogacy legislation, as this can simplify both the legal process and insurance landscape. Finally, your surrogate's employment status and income level will determine her lost-wage compensation, and her level of prior surrogacy experience will influence her base compensation.

Escrow Reminders & Financial Management Surcharge

Administrative oversight fee to review receipts, approve monthly surrogate line-item requests, and send fund reminders.

Clinical & Medical Care Management

Specialized coordination services for medical milestones and complex clinical needs.

IVF Clinic Setup & Coordination

Covers the labor of syncing the surrogate's local monitoring clinic with your primary IVF clinic, coordinating medical record transfers, and tracking the medication schedule.

Maternity Care Coordination

$3,000 – $5,000

A milestone fee charged once pregnancy is confirmed. Covers managing the transition from the IVF clinic to the local OB/GYN, tracking prenatal appointments, and coordinating hospital delivery logistics.

Medical Specialist Advocacy

$250 – $350/hour

Activated if the pregnancy requires high-risk specialists (Maternal-Fetal Medicine), if the baby requires a NICU stay, or if a dedicated caseworker must advocate for the intended parents' rights at an uncooperative hospital.

Escrow Funds

Approximately $100,000 – $120,000

Escrow funds are held by an independent escrow company and disbursed throughout the journey to cover surrogate compensation and related expenses.
